In order to help us provide quality service to you, please have the following ready during your visit:

- List of medications you may need
- New patient form (optional, but preferred if you’re a new patient)
We accept most insurance carriers and health care plans including PPO, HMO, and private insurance. If you have any doubts or further questions, feel free to give us a call. Here are some of the insurance carriers that we accept:

Only if it is urgent to be seen on the same day.
With an appointment, the wait is approximately 0 to 15 minutes. For walk-ins, the wait is approximately 30 minutes.
We have Mandarin, English, and Spanish speakers in the office that can cater to your needs. Specifically, Dr. Yuan Fei Chang is fluent in Mandarin and English.
The stamp charge allows us to send you your lab reports and saves you an extra trip to the doctor just to pick up your reports.
We accept cash or check only.
We refill prescriptions via phone, email, and fax.
